Dr. Austin Smith saw it every clinic day: the detail from each visit got lost between EMR screens, and what made it into the note was free text he couldn’t analyze. He studied engineering before he became a shoulder surgeon, so he built the fix himself, and he has charted with it since his first months in practice.
