LastPatient Capture

Every fact, captured at the visit.

Work the way you already do. Capture can listen to the visit with the patient’s consent, or you type, click and dictate. LastPatient’s database structure defines the variables, and your dot phrases add your own. Each fact lands in its own as you go. You confirm them before you close the note, and it’s written from what you confirmed.

From visit to variablesSeven sources from one knee visit (an MRI report, the patient saying the knee locks on stairs, the dictated diagnosis, an exam measurement, the surgeon’s plan, a patient survey and published evidence) each turn into separate variables: MRI result, locking, trigger, diagnosis, side, flexion, plan, PT duration, surgery considered, the condition for surgery, pain score, sleep, evidence and matched studies.IN THE ROOMAFTER THE ROOMCHARTMRI report: medial meniscus tear.HEARD“It locks up when I take the stairs.”DICTATED“Left knee, medial meniscus tear.”EXAM“Flexion, left, 110.”AGENTIC CHECKOUT“Let’s try PT for six weeks. What it does

What happens during the visit.

Listen, type or dictate

Capture can listen with the patient’s consent. You can also type, click, dictate or use your dot phrases.

Variables defined as you go

LastPatient’s database structure defines the variables, and your dot phrases add your own. Side, structure, findings and plan each land in their own.

Confirmed by you

You review and sign before you close the note. A person checks every note, every time.

Documenting the visit

The note is done before you walk out of the room.

  1. The visitListened to, typed or dictated
  2. DefinedEach fact, its own variable
  3. ConfirmedBy you, before the note closes
  4. The noteWritten from the variables

The usual note, one block of text

“54-year-old male, right-hand dominant. Full-thickness supraspinatus tear, arthroscopic repair with two anchors. No biceps tenodesis…”

Before any of this is usable, software has to re-read it and guess.

Variables you define

Every blank is its own variable.

LastPatient’s database structure defines the core variables, and your dot phrases add your own. Every blank in a dot phrase is its own variable: type .kneeexam, fill in side and flexion, and both are saved as data, not just text. Update a dot phrase once and every dot phrase that nests it picks up the change, so there’s no going through them one by one.

The dot phrase

.kneeexam

{{side}} knee: ROM 0 to {{flexion}} degrees. {{.stability}}

  • Blue = blanks you fill in.
  • Teal = another dot phrase.

What it saves

Left knee: ROM 0 to 120 degrees. Stable to varus and valgus stress.

  • SideLeft
  • Flexion120°
  • StabilityStable, varus and valgus

Questions surgeons ask

What surgeons ask us first.

Does it replace my EMR?

No. It works alongside your EMR, as an overlay that interacts with it through an API.

Where does the audio go?

It’s transcribed on the device. The Security page has the rest.

How many variables can I capture?

LastPatient’s database structure sets the core variables. On top of those, every blank in your own dot phrases adds one, with no fixed limit.

Every fact, captured at the visit.

We’ll show it to you on your own visits.
